The first step starts with call us to come out for an assessment.
A qualified registered comes out and does a head-to-toe assessment. Your physician sends a referral to the hospice. The Nurse and our Medical Director review the patient's medical history and their current medication regimen.
If the patient qualifies for end-of-life care related to having a terminal illness the patient is admitted into the hospice care program.
